LifeMed

This latest LifeMed app from NewWave Health Sdn Bhd is for patients suffering long-term or chronic conditions.

LifeMed is a digital health service for patients that are registered with a participating hospital or clinic and are diagnosed with Type 2 Diabetes or Hypertension. It empowers patients to manage their own condition from the comfort of home, work, school, holidays or anywhere.

PLEASE NOTE: In order to use this app you must firstly be assigned to the LifeMed programme by your participating healthcare provider, hospital, doctor or clinician, and be in possession of a valid LifeMed username and authentication provided by your healthcare provider.

This version of the LifeMed app enables patients already diagnosed with Type 2 Diabetes or Hypertension to monitor their blood glucose levels, blood pressure and general well being without having to visit the health centre.

The data captured via this app is uploaded to the LifeMed Clinical Portal at your participating hospital, where qualified doctors can monitor your progress and make timely interventions if required. This might include changes to diet or exercise, help or advice or even modifications to medication and arranging clinic appointments.

Please note that this is not a 24x7 monitored service and is not used for identifying emergency treatments. If you are feeling unwell you must not rely on the app and contact your emergency services as soon as possible.

To become a patient, please contact your healthcare provider and if they are participating in the LifeMed program, and you qualify to use LifeMed, you will then be provided (fees apply) with a LifeMed patient account and a Bluetooth glucometer or a Blood Pressure meter. You will also receive training materials and user guides.

You will then be asked to use the app to take regular readings which may be several times per day, daily, weekly or longer depending on your particular circumstances and what your doctor advises.

These will be uploaded to your healthcare provider where the data can be used to assess your progress on a regular basis (not in real-time) by qualified clinical staff. Your doctor will inform you of how frequently your readings may be assessed. Do not rely on this service for emergency situations as it is only designed as a monitoring tool to check your illness is under control.

Home monitoring with LifeMed empowers you, the patient, to take control of managing your own condition. It gives peace of mind and enables you to get on with your everyday life without the need for regular clinic check-ups.
